Membership Application
IMPORTANT MEMBERSHIP INFORMATION, PLEASE READ:
Eligibility:

Any individual employed by Methodist Le Bonheur Healthcare or one of its affiliates is eligible to join the credit union (for a complete list of MLH affiliates, click here).

Membership may be retained with good account standing, even if the associate changes employment, retires, or moves.

Spouses or children may become members and have separate accounts independent of the associate's membership.

Any individual eligible to join the credit union may do so by submitting a membership application to any Methodist Healthcare Federal Credit Union (MHFCU) branch.

MHFCU requires a minimum of two forms of identification (one each from the Primary and Secondary columns listed below), including at least one form of photo identification, to verify identity. The following documents are acceptable:

**If the address on your ID does not match the address you would like on your account, you must provide a piece of mail as proof of current address.
Costs & Requirements:

Any individual eligible to join the credit union must pay a $0.25 one-time membership fee and subscribe to at least one $5.00 share (savings account).

Five Dollars ($5.00) must remain on deposit at all times to keep the account open, and the account must show financial activity during every 12 months to keep the account from reaching a dormant status.
